Fortis provided a summative evaluation of the Partners in Change training program, a patient-centered service for disability and mental health, for Avivo,

Avivo is a recognised leader in innovative approaches to consumer-directed care and individualised service design and delivery – in the disability, aged and mental health sectors. Our role was to undertake a summative evaluation of the Partners in Change program.

Partners in Change is an Avivo training project funded through two FaHCSIA programs: the Mental Health National Respite Development Fund and Respite for Older Carers of Children with a Disability Program.

The project aim was to expand the underpinning knowledge and skills of participants to deliver patient-centred services in the mental health and disability sectors.

Services Provided:

Fortis coordinated a desktop review, scrutinising existing databases and reports, as well as conducting individual interviews with course facilitators, developers, senior personnel involved in the management of the program and course participants.

Project Outcomes:

The major outcome for the client was a 35-page report analysing the effectiveness of the Partners in Change initiative, the ongoing impact on staff that had participated and the people/patients they support.
